WHAT DO I NEED TO TRY OUT?

Any student-athlete interested in trying out for any of the Lewis Mills Athletic teams will need the following items:

    1. Download and Read the Student-Athlete Handbook

2. Blue Card or Valid Red Card issued and signed by the School Nurse

3. Signed Athlete Policy and Medical Authorization Forms

4. Signed Notification of Online Access to the Student-Athlete Handbook

Prior to actually participating in any physical activity, conditioning or tryout with any Lewis Mills team, all athletes are required to submit a Blue Card to their respective coach. Blue cards are issued by the school nurse and establish proof of a current physical being on file. More information on Blue Cards is available here. Physicals are good for 13 months from the date of the physical. In the event a physical is going to expire prior to the completion of the season, the High School Nurse will issue a RED Card. If an athlete receives a Red Card, it is their responsiblity to update their physical information with the Nurse and obtain a Blue Card to present to the coach.

In addition, athletes and their parents must complete and sign a Lewis Mills Athletic Policy form and Emergency Medical Authorization form and turn these into the coach prior to trying out for the team. These forms may be turned into the Athletic Office no later than one week before the first day of tryouts. Copies of these forms are available from the links above or may be obtained directly from the Athletic Office by request.

Tryout dates and times will be published on the Lewis Mills Athletic Department Homepage when they become available. Tryouts for all Lewis Mills teams are open and do not require advanced notification to the coaching staff.

No student-athlete may tryout for a team unless all of their forms have been complete and turned in either to the Athletic Office (early) or the Coach (1st Day of Tryouts)
