Secondary Athletic Insurance Information:
Lewis Mills High Schools
Interscholastic Sports Insurance Policy is a secondary or “excess”
benefit plan. All claims must first be submitted to the individual’s
personal insurance carrier. Once the primary carrier has assigned its benefits,
the school’s plan will review the claim. The athletic insurance will
pay only the usual and customary portion of the balance of the medical expenses
not provided or reimbursable under the individual’s primary coverage.
Claim forms must be submitted with 90 days from the accident date.
