BLUE CARD INFORMATION

Prior to the first tryout/practice session, all candidates for athletic teams are required to be examined by their family physician, a walk-in clinic physician, or the school physician. These completed physical examination forms (or copies of) must be on file in the Health Office of the high school prior to the first tryout/practice session.

Students are responsible for obtaining a signed Blue Card from the school nurse. This Blue Card serves several purposes and will need to be turned into their coach before they will be allowed to tryout or practice with the team in any way. For students entering 9th grade, the Har-Bur Nursing Staff will issue Blue Cards for the following fall sports season towards the end of the school year for any 8th graders requesting one as long as their physical does not expire during the fall or sports season. If the physical expires prior to the completion of the fall season, the students will need to get there Blue Cards from the High School Nursing Staff. In the event a physical is going to expire prior to the completion of the season, the High School Nurse will issue a RED Card. In the event an athlete receives a Red Card, it is their responsiblity to update their physical information with the Nurse and pick up a Blue Card to present to the coach.

This card actually provides the coach with more information than simply a proof of physical. In addition, it will also provide the coach with the date that the athletes physical expires. Should the physical expire during the season, the athlete will immediately become inelligible to participate unless a new physical exam is completed by a physician and turned into the nurse and a new Blue Card is issued.
Parents and Student-Athletes are responsible for ensuring that the sports physical is up-to-date.

The sports physical examination (Download a form here for your physician) is valid for 13 months. Lewis S. Mills High School offers three (3) free sports physicals each year; the first prior to the winter season, the second prior to the spring season and the final one towards the end of the school year for the following fall. Please check with the school nurse for the date of the next scheduled free sports physical.

The Blue Card finally provides the coach with one more extremely important piece of information. Any special restrictions or serious medical concerns that the coach needs to be aware of will be noted on this card. This is often information that is not readily understood on the physical signed by the physician. The coaches are required to keep these cards on file for the duration of the season as a reference.
