Case Manager - Collaborative Model

The case manager is a Collaborative Team member assigned responsibility for specific identified students based on their disabilities. The case manager consults with the classroom teacher regularly and maintains communication with the identified student's parents. In the collaborative model the case manager provides support for the identified student in the classroom and provides individiualized instruction when needed in a separate setting.Referral Process

When a classroom teacher has concerns about a student's progress she/he makes a referral to the Student Assistance Team which recommends strategies designed to assist the student's learning. If the student continues to show slow progress then a referral may be made to the Special Education Department. When a referral has been received, the Special Education Coordinator will arrange a Planning and Placement Team Meeting to review the referral and to plan assessment (see handbook for further details).Assessment Battery

A variety of tests are available to assess a student's strengths and weaknesses. Each specialist recommends the tests to be used according to the needs of the student. Generally, assessment includes a measure of the student's intellectual ability, level of academic achievement, level of language competence and information processing capabilities.Acronyms
