Welcome to the Har-Bur Middle School Health Room.  Your school nurses are Mrs. Mauro 5^th and 6^th grade, and Mrs. Niedmann  7^th and 8^th grade. This web page is designed to educate you about the services offered at Har-Bur,  state mandated requirements for middle schoolers and health and safety education. We appreciate any medical updates with your child to keep us better informed. If your child is going to be out sick or will be arriving late to school, please give us a call and let us know. To report the illness and/or absence of a 5^th or 6^th grader, call Mrs. Mauro at 673-6163 ext.7310. For 7^th and 8^th graders  call Mrs. Niedmann at 673-6163 ext.7507. PHYSICALS In accordance with Connecticut State Law, a health assessment is required prior to public school enrollment and in grade 7 and 10. A physical examination includes height, weight and blood pressure, hematocrit/hemoglobin, vision, hearing, speech and gross dental screenings, updating of immunizations and chronic disease assessment. The health assessment information MUST be on the State of Connecticut Department of Health Assessment Record(form HAR-3). PLEASE BE ADVISED, YOUR CHILD WILL NOT BE ALLOWED TO START EIGHTH GRADE UNTIL A COMPLETED PHYSICAL EXAMINATION DONE IN EITHER 6^TH OR 7^TH GRADE IS PRESENTED TO THE NURSE'S OFFICE. *Click here for blue State of Connecticut Health Assessment Record Form SPORTS PHYSICALS Region 10 sports physical exam policy states: Students wishing to participate on any middle school or high school athletic team shall have a physical examination prior to participating in the first practice for his/her first sport each year. A general physical examination (blue State of Conn. Form) will be accepted in lieu of the regular sports physical in any one calendar year. As an alternative to having a physical by your own physician, we offer sports physicals in the health room at Har-Bur given by our school physician, Dr. Liftman. Please note, the sports physical does not satisfy the requirement for a complete physical on blue State of Connecticut Assessment Record for entrance into 8^th grade. IMMUNIZATIONS MMR 2^nddose for entry into 7^th grade HEPATITIS B 1 dose for entry into 7^th grade, complete the series of 3 doses for entry into 8^th grade VARICELLA 1 dose on or after 1^st birthday or verification of disease(chicken pox). Required for entry into 7^th grade Students will not be allowed to start school without documentation of required immunizations unless the student is exempt for religious or medical reasons. ABSENCE FROM SCHOOL If your student is out ill, or will be late to school, please call the health office 673-6163x7507. Please send a written excuse with your child when they return from illness. If an extended period of absence of 2 or more days is expected, parents/guardians may call the nurse in the morning to report the absence and request homework. It is the responsibility of the parent/guardian to pick up the assignments. It is recommended that a student remain home if any of the following conditions are present: FEVER. The student should remain home with a fever greater than 100 degrees. The student can return to school after he/she has been fever free for 24 hours(without fever reducing medicine such as Tylenol or Motrin). DIARRHEA/VOMITING. A student with diarrhea and/or vomiting should stay at home and return to school only after being symptom-free for 24 hours. CONJUNCTIVITIS(pink eye).Following a diagnosis of conjunctivitis, the student may return to school 24 hours after the first dose of prescribed medication. RASHES. Common infectious diseases with rashes are most contagious in the early stages. A student with a suspicious rash should return to school only after a health care provider has made a diagnosis and authorized the student's return to school. A student who has started antibiotics needs to be on the medication for 24 hours before considered non-contagious and able to return to school. When a student returns to school, he/she should be able to participate in all school activities including physical education. MEDICATION ADMINISTRATION Every effort should be made to have any medications given at home. If this is not possible, remember that ALL medications, including non-prescription over the counter medications require written authorization from a doctor and parent. State law and regulations pertaining to administration of medications in school requires an Authorization of Medication By School Personnel form to be completed by a doctor, dentist or Advanced Practice Registered Nurse. The form must also be signed by a parent or guardian.
